Abstract

Osteoarthrosis of the knee-joint is a frequent cause of severe disability in the elderly patient. It results from
degeneration of the articular cartilage and may be accelerated
(1 X
by trauma, excessive use or abuse and obesity. It tends to progress with age and many patients who have minimal complaints during their earlier active years become severly disabled when they reach 60 or 70 years of age. ’’
The patella being part of the articulation of the knee joint is affected by any supervening pathology. On it lies
the thickest cartilage in the body and perhaps not surprisingly
(2 }
the most frequent site of cartilage degeneration. ’Usually,
the affection involves the lateral patellar facet more than
the medial and there are usually mirror changes on the opposing

femoral condyle.
The presenting features of patello-femoral osteoarthrosis are swelling, stiffness, pain referred to the anterior aspect of the knee or medial or Lateral to the joint line. Climbing stairs may become impossible. Giving way while walking is not frequent because the patient is often cautious. Sudden giving way while rising from a chair or descending stairs, associated with sharp pain is common.Crepitus may be audible. The patella may be obviously laterally subluxed. Transverse passive movement is usually limited. Flexion^of theiknee
