Abstract In the present work. we studied the effect of the high-ceiling diuretics on serum uric acid in patients with oedema due to different causes. Fu~osemide was used as an example of the high-ceiling diuretics and the study was carried out on 45 patients with oedema. Those patients were classified into three groups. each of 15 patients. The first group comprised patients with oedema due to liver cirrhosis. the second group comprised patients with renal oedema. and the third group comprised patients with oedema due to congestive heart failure. 15 norma L individuals 0 f the same age range and sex were taken a~, a control group. Furosemide is structurally related to the thiazides. but its principle site of action is in the ascending limb of the loop of Henle. It probably also has effects along the entire nephron with the exception of the distal aldosterone sensitive portion. Furosemide is efficient in the presence 0 f glomerular filtration rates of 10 ml/minute or less as in severe heart failure and renal failure where other diuretics fail. The patients and the control group were given two tablets of furosemide i.e 80 mg for one week and serum uric acid was estimated before and after receiving furo sem ide. --- ---- ------ 87 Serum uric acid may be raised because of a diminished tubular secretion of uric acid. an increased endogenous production of uric acid or a high protein intake. ___ The first group of patients with liver cirrhosis showed a highly significant increase in the serum uric acid compared to the control cases before receiving furosemide . After receiving furosemide. there was a significant increase of serum uric acid in these hepatic cases compared to the level before furosemide. The second group involving patients with renal failure showed a highly significant increase in serum uric acid compared with control cases before furosemide. After furosemide in that group of patients with renal disease. a highly significant increase in serum uric acid level was observed compared to control cases after furosemide. In the last group which included patients with congestive heart failure. an insignificant increase of serum uric acid level before furosemide was observed compared to its level in the control q roup , A highly significant increase in the serum uric acid was observed after receiving furosemide in comparison to the control group after furosemide. It is thus concluded that ingestion of many diuretics particularly furosemide. causes a rise of serum uric acid. ------ 88 It is thus important in ~atients receiving diuretics especially furosemide, to observe their serum uric acid sO as to protect them against the hyperuricemic effect of these drugs. |
