Abstract

Lymphomas are neoplasms originating from lymphoproliferative tissue. There are two broad groups of lymphomas: Hodgkin’s disease and non Hodgkin’s lymphoma .The former is characterized by the prescence of reed-sternberg (R.S) giant cells in a background of non neoplastic inflammatory cells which are absent in the later. The incidence of lymphoma is on increase and non Hodgkin’s lymphoma is considered one of the most prevalent tumor all over the world with mean incidence of about 8.1 %. In Egypt it comes third after bladder and breast carcinoma.The diagnosis and sub classification of malignant lymphomas are complex and often rely on subtle difference in morphology that are subjected to interpretation so immunohistochemistry and gentic studies are often very helpful in establishing correct diagnosis. Indolent lymphomas constitute heterogenous group of lymphoproliferative disorders and usually associated with relatively good prolonged survival. There are many classes of genetic alteration including chromosomal translocation, gene amplification, gene deletion, point mutation and viral inseration into host genome. Most of these genetic alterations appear to be somatically acquired rather than inherited and they result in inappropriate constitutive activation of proto-oncogenes and their transformation into oncogenes that permit an otherwise normal cells to divide independently of normal growth regulatory mechanisms or alternatively inactivation of anti-oncogenes ,tumor suppressor genes. This work was done on 464 cases of lymphoma diagnosed in pathology department, Mansoura faculty of medicine & pathological laboratories they are revised according to the classification introduced by international lymphoma study groups for low grade B cell lymphoma. The paraffin blocks were cut at 4-5 micron then stained by haematoxylin and eosin stain, reticulin stain & mmunohistochemical stain. non Hodgkin lymphoma represents 74.5% while Hodgkin lymphoma represents 25.5%. Also the relative frequency of indolent lymphoma is 13% & in contrast that of aggressive lymphoma is 87%. Small lymphocytic lymphoma represents 46.6 % of indolent lymphoma while follicular lymphoma represents 26.6% of indolent lymphoma. Marginal zone lymphoma represents 20% of indolent lymphoma while mantle cell lymphoma is 4.4% of indolent lymphoma & finally hairy cell leukemia represents 2.2% of indolent lymphoma. The epidemiology of indolent lymphoma in Egypt should be extensively studied by more investigations at different levels to reach the exact incidence of this lesion. Pannel of monoclonal antibodies must be applied in all cases of non Hodgkin lymphomas formed of diffuse infilterate of small lymphoid cells & this pannel include CD5, CD23, CD103, Ig M, Cyclin D1 & Bcl- 2.
