Abstract

Diagnoses of hidden hematologic abnormalities among emergency and critically ill patients is crucial for the evaluation of the course and outcome of these patients. Our goal from this thesis was to study the spectrum and prevalence of hematologic abnormalities in emergency cases and to plan a simple protocol to assess the emergency patients from the hematologic point of view. To achieve this goal a cohort of 400 emergency patients presented to Al­Mansoura University Emergency Hospital with different medical and surgical emergencies were enrolled. They were 216 males and 184 females.For each patient and control, hematological assessment entailed the performance of CBC, ESR and the examination of peripheral blood smear. Hemostatic parameters included both PT and APTT. Important biochemical tests done were ALT, AST, albumin, total bilirubin, creatinine and RBG.The most important findings were, abnormal Hb (anemia in particular) was met with in 2/3 of all emergency patients in our study. More than a half of all these patients also had abnormally high (or less commonly abnormally low) WBC count. Neutrophilia was the most frequent abnormality in white cell series (particularly in surgical patients; p=0.0001). Abnormal platelet counts (mostly thrombocytopenia) were encountered in more than a quarter of all emergency cases. This thrombocytopenia should be seriously regarded since PT was abnormal in more than a half of our patients (specially in medical patients; p=0.0001) and APTT was also involved in 22.7% of all patients (also mainly in medical patients; p=0.04). Our conclusions are, there was a high prevalence of hematologic abnormalities in patients mitted to Al­Mansoura Univesity Emergency Hospital with different medical and sugical emergencies. Abnormal PT was observed in 51.5% of all emergency patients (particularly medical cases). This should be regarded serious particularly in the presence of other hemostatic defects. Lastly, Biochemical abnormalities (covering liver and kidney functions and RBG) were much less prevalent in emergency patients as compared to the high frequency of hematologic and hemostatic problem.
