Abstract

Purpose : Evaluation of changes in contrast sensitivity as an early diagnostic procedure in response to increased intra-ocular pressure. Material & Method : Ninety-eight eyes of fifty-one persons were classified into 4 groups . Group1 : Primary open -angle glaucoma group (n: 37 eyes) .Group 2 : Glaucoma suspect group (n : 22 eyes) . Group 3: Normal-tension glaucoma group (n: 19 eyes). Group 4 : Control group ( n : 20 eyes ) .Patients were followed up for anterior segment examination , ophthalmoscopy , intra ocular pressure measurament and contrast sensitivity testing using Mesotest IIb every month . Visual field examination by Humphrey Field Analyzer every 3 months and measurement of the NFL thickness by Optical Coherence Tomography every 6 months . Results : Contrast sensitivity testing is statistically highly significant in differentiating between primary open-angle glaucoma, glaucoma suspect, and normal-tension glaucoma in comparison to control groups. The POAG group has lowest median contrast sensitivity and NTG group shows lower median contrast sensitivity. However , there is weak correlation mostly inverse correlation between contrast sensitivity and intra-ocular pressure. Other interesting findings, the demonstration of weak positive correlation between visual field mean deviation and contrast sensitivity within all groups. Also , there is weak positive correlation between retinal nerve fiber layer thickness and contrast sensitivity in all groups. Conclusion : Contrast sensitivity can be a useful component of a clinical evaluation in association with OCT and visual field in early diagnosis of glaucoma cases . Contrast sensitivity can be used to establish a diagnosis of suspected glaucoma and normal tension glaucoma in the presence of optic nerve changes in the absence of diagnostic visual field changes.
