Abstract

Adult patients with autoimmune markers at diagnosis of diabetes have a progressive decline in beta cell function and have a very high frequency of future need for insulin. All patients should therefore be offered a test for autoantibodies at the time of diagnosis of diabetes. Phenotype characterization alone is not adequate for correct classification; knowledge of pancreatic autoantibodies gives significant additional information. This study was conducted on 72 diabetic patients with age range 35­39 years and were firstly diagnosed as being diabetic at the same age range. Patients with LADA had: lower BMI, lower systolic& diastolic blood pressure, lowerHDL cholesterol and lower prevalence of peripheral neuropathy. As the importance of autoantibody status is now emphasized in the new guidelines for diagnosis and classification of diabetes. It is recommended that GAD­Ab be tested for, since GAD­Ab is the most common antibody and has the strongest influence on further beta cell damage. However, since some LADA patients have an isolated ICA, it is preferable to test for more than one autoantibody if possible.
