Abstract

In patients with mature anterior teeth with necrotic pulp and periapical radiolucency, this study was done to evaluate the regenerative potential of those teeth using different disinfection protocols. The efficacy of revascularization was evaluated by
• Resolution of periapical radiolucency: measurement of periapical index (PAI) scores before and after the regenerative endodontic procedure (REP).
• Regaining pulp sensibility: Recoding electric pulp tester (EPT)
reading before and after the REP.
thirty-six patients with mature necrotic anterior teeth with periapical lesions were randomly allocated into 3 equal groups (n=12): group A; was treated by conventional TAP disinfection technique, group B; disinfection was done using XP-finisher activation and group C; disinfection was done using diode laser. After REP in all groups, follow up was done for one year. Periradicular healing was evaluated through standardized radiographs taken at baseline, 6, and 12 months. EPT readings were recorded at baseline and during follow-up to assess if the teeth in all groups regained sensibility during the follow-up period. Statistical analysis was done using Kruskal-Wallis test followed by Dunn’s post hoc test with Bonferroni correction for intergroup comparisons and Freidman’s test followed by Nemenyi post hoc test for intragroup comparisons. The significance level was set at p≤0.05 within all tests. Statistical analysis was performed with R statistical analysis software version 4.1.3 for Windows.
The results showed that there was a statistically significant decrease in PAI score in all groups at 6 and 12 months of treatment.
Regarding the effects of time intervals, study demonstrated a statistically significant decline in PAI score after six months, while the lowest value was shown in the Laser group after a year.
In terms of regaining tooth sensibility, the current study showed a statistically significant reduction in EPT score in all groups after 6 months of treatment, indicating regaining tooth sensibility.
A statistically significant difference was found between EPT readings in the 6th and 12th months in all groups while group (C) had the lowest EPT score after one year of follow up.
