Abstract

About 80 years ago, abnormally invasive placenta was first explained as a clinico-pathological condition in which the placenta does not completely separate from the uterine wall (Hertig, 1937). There are several theories as to why that occurs, the most supported being a failure of normal decidualization in a uterine area with a defect in the endometrium -myometrial interface, typically at the site of previous cesarean scar(Einerson, 2020).
In Egypt, there has been a 10-fold rise in the incidence of abnormally invasive placenta since the 1970 according to a study done in El-Sayed Galal Hospital (Ibraheem, 2018). This has mainly been attributed to increase in rates of cesarean sections.
This is a pilot study. The main objective is data gathering to determine the correlation between different risk factors and the increasing incidence rates of abnormally invasive placenta in Egypt.
The present study was done in Kasr El Aini Obstetrics and Gynecology Hospital during a six-month period from November 2021 to April 2022. The risk factors evaluated are: maternal age, number of previous cesarean and other uterine surgeries, the regions in which those surgeries were preformed, the type of medical facility in which they were preformed, level of the surgeon that performed the surgery, and any complications that arose from those surgeries including wound infections, post operative endometritis, post operative exploration, need for blood transfusions and injury to bladder, ureter or bowel.
All patients included in the study were subjected to history taking and US examination. Admitted patients that are suspected of having abnormally invasive placenta by 2D US were followed up to confirm their diagnosis of PAS by post- operative histopathology.
Data gathered from the confirmed PAS patients was analyzed for correlation using p value. In this study, the mean age of the collected sample was 33.4, with ages ranging from 23 to 43 years. The age of the patients was found to be statistically significant with the larger number of patients found in the “below 35 years of age” category. No other factors were found to be statistically significant.
Pilot studies can guide in the design and implementation of larger scale studies regarding PAS that would give more conclusive results
