Abstract SUMMARY Ulnar neuropathy at the elbow (UNE) is considered the second most common compressive neuropathy in the upper extremity, resulting from compression and traction on the ulnar nerve (UN) at the elbow. Electrodiagnostic studies aid in establishing the diagnosis, however, there is general agreement that electrodiagnostic testing cannot diagnose a significant number of UNEs. Ultrasound (US) is particularly helpful in this regard. Perineural corticosteroids (CS) and platelet-rich plasma (PRP) injections have been widely used for the management of patients with CTS. However, no other previous publication studied the efficacy of perineural PRP injection and only a few studies used perineural CS injection in management of UNE. The aim of this study was to assess and compare the therapeutic effects of ultrasound-guided deep perineural PRP and CS injection in patients with ulnar neuropathy at elbow. This study was performed on 40 patients with mild to moderate ulnar neuropathy at elbow diagnosed clinically with either electrophysiological and/or ultrasonographic findings. They were randomly divided into 2 groups (20 patient in each group): Patients of group I received a single ultrasound-guided deep perineural injection with 3ml PRP at the thickest part of the UN. Patients of group II received a single ultrasound-guided deep perineural injection with 1ml CS (triamcinolone acetonide, 40mg/mL) mixed with 1ml lidocaine hydrochloride. Any regular treatment taken by patients for the condition was stopped before intervention until the end of follow-up period. |
