Abstract

To evaluate and compare the success rate, intracanal bacterial count, sensitivity and tooth discoloration following single visit versus two visit regenerative endodontic procedures in non-vital mature anterior teeth with periapical lesion.
Methods: Thirty-two patients were included in this study. After confirming the diagnosis clinically and radiographically, patients were randomly assigned into two equal groups. In the two groups, at the first appointment, each tooth was isolated with a rubber dam followed by access cavity preparation. Three successive sterile paper points were used to obtain the first bacteriological sample (S1). The canal was instrumented and irrigated by 20 ml of each 2.5 % NaOCl and 17% EDTA, for the intervention group, the post instrumentation bacteriological sample (S2) was collected from the canal and sent to the microbiology laboratory, PRF was prepared and placed into the root canal followed by placement of biodentine. Pulp chamber was permanently restored using resin composite. For the control group, calcium hydroxide was placed as an intracanal medicament. In the second visit after 2-3 weeks, the post instrumentation sample (S2) was collected after removing the intracanal medicament and irrigation of 20 ml 17% EDTA, then regenerative procedures were carried out in the same manner. Patients were recalled for follow up visits at 3, 6, 9 and 12 months. The success rate clinically (pain, sinus & swelling) and radiographically (healing) was evaluated at 3, 6, 9, and 12 months. For the secondary outcome, the intracanal bacterial count was determined using the bacterial culture technique. Sensitivity was evaluated using an electric pulp tester, incidence of discoloration was measured by using VITA 3D master shade guide. All demographic and outcome data were collected and statistically analyzed.
Results: The overall success rate was 28.6 % in the intervention group, while the control group showed a success rate of 64.3% with no statistically significant difference between both groups. Both study groups showed a significant reduction of bacterial count after the irrigation step with no statistically significant difference between both groups. Regarding the sensitivity and discoloration, there was no statistically significant difference between single visit and two visit regeneration groups
