Abstract

Prostatic carcinoma is the second most frequently diagnosed cancer worldwide and is the sixth leading cause of cancer deaths in men, yet it varies greatly in its aggressiveness. There is an urgent need to identify markers of cancer prostate progression and invasiveness to be more accurate in predicting prognosis. This study aimed to assess immunohistochemical expression of TWIST in BPH, PIN and prostatic carcinoma, using 48 archival prostatic tissue samples. Prostatic carcinoma was detected in all studied specimens. There were adjacent BPH lesions in nineteen specimens, PIN in one specimen and both BPH and PIN together in three specimens in addition to prostatic carcinoma lesions. The Gleason score in carcinoma lesions ranged from 6 to 10. There was a significant increase in TWIST expression observed in prostatic carcinoma (moderate in 16.7% and marked in 83.3% of lesions) and PIN (mild in 25%, moderate in 25% and marked in 50% of lesions) versus BPH (negative in 59.1% and mild in 40.9% of lesions). This means that TWIST expression can be used to differentiate between prostatic carcinoma and BPH in problematic cases (p value < 0.05) but not between prostatic carcinoma and PIN (p value >0.05). There was a lack of correlation between Gleason scores and TWIST expression (p value >0.05). This supports the accumulating evidence that TWIST plays an important role in the pathogenesis and evolution of prostatic carcinoma rather than the prognosis, thus TWIST is not a reliable prognostic factor in prostatic carcinoma
