Abstract

Osteoarthritis is a major public health problem. Neuromuscular training has been increasingly applied for patients with osteoarthritis.
Purpose of the study is to compare the effects of neuromuscular training vs. strength exercises on decreasing pain, reducing disability level, improving physical functional activity, and improving muscle strength on patients with knee osteoarthritis, by using VAS, WOMAC, TUG, 6MWT, and HHD.
Methods A sample of fifty one patients with knee OA their ages ranges 45–65 years and their BMI 18–30 kg/m2 participated in this study, they were divided randomly into 3 equal groups (17 patients). group Ⅰ [Control group], received TENS and traditional exercises for 12 sessions. group Ⅱ [strength group] received same as group one plus strengthening exercise program. group ⅠⅡ [Neuromuscular group] received same as group Ⅰ plus neuromuscular training program. All participants were evaluated before and after 12 sessions on alternative days through measuring VAS, WOMAC, TUG, 6MWT andHHD to evaluate pain, disability level, physical activity, and muscle strength.
Results The results of the study reviewed that there was a significant main effect of treatment. There was a significant decrease in VAS, WOMAC and TUG in the three groups post treatment compared with that pre-treatment within groups. There was a significant increase in 6MWT in the three groups post treatment compared with that pre-treatment. Between groups comparisons pre-treatment revealed a no significant difference in all parameters. Comparison between groups post treatment revealed a significant decrease in VAS and WOMAC of group II and group III compared with that of group I. while there was no significant difference between group I and II. Also there was no significant difference in TUG and 6MWT between the three groups post treatment. About HHD there was a significant increase in quadriceps strength of the group IIand III compared with that of group I. And there was no significant difference in quadriceps strength between group II and III. And for hip abductors muscles there was a significant increase in hip abductors strength of the group II and group III compared with that of group I. There was no significant difference in hip abductors strength between group II and III.
Conclusion there are no significant differences between the effect of Neuromuscular training (NMT), and strength exercises on decreasing pain intensity level in VAS, improving function of disability in WOMAC, improving functional mobility in TUG test, improving performance in 6MWT in patients with knee osteoarthritis (KOA), and increasing quadriceps and hip abductors muscles strength in HHD.
