Abstract

Background: Global strain by speckle tracking echocardiography (STE) is emerging as a reproducible quantitative means to assess left ventricular function. Our aim was to determine a simple regression model formula to estimate LV GLS and ejection fraction (EF), echocardiography.
Methods: We studied 224 consecutive patients with inherited heart muscle disease, 124 patients with HCM and 104 patients with idiopathic dilated cardiomyopathy. Patients were compared with 30 age and sex matched healthy individuals. who had both CMR and STE, 2D strain imaging was applied to asses segmental and global longitudinal strain (GLS) and strain rate.
Results: In HCM GLS and LV SRsys were inversely correlated with MWT, septum, LVPW, LVM, LVMI and E/e’. LV SRe was inversely correlated with MWT, septum and LVMI in HCM group. In IDCM GLS did not show significant correlation with any of conventional echocardiographic variables. However, LV SRsys was inversely correlated to LA diameter, LA volume index, ESD, EDD, EF%, LVM, LVMI, MR and Mitral E/A in DCM group. SRe was directly correlated with EF% (P<0.001) and inversely correlated to LA size, LA diameter, LA volume, LA volume index (P<0.006).
Multiple regression analysis was used in each group to detect the independent variables that modulate and affect LV GLS (dependent variable). In IDCM: LV GLS = (-1.823) + (-0.133) EF + (0.109) EDD +(-0.062) SBP (if EF is introduced as %; e.g. 33%). While in HCM: LV GLS = (-9.134) + (-0.101) EF + (0.057) LA diameter +(0.004) LVMI + (0.039) EDD + (0.414) MR.
