Abstract

Background and objectives:Statins inhibit 3-hydroxy-3-methyl glutaryl-coenzyme A (HMG-CoA) reductase enzyme and thus decrease biosynthesis of cholesterol and Coenzyme Q10 (CoQ10) as well. Therefore, Statins lead tomuscular mitochondrial dysfunction and myopathy. This work was planned to study and compare the possible protective effect of CoQ10versusAlfacalcidol (ALF) in atorvastatin (Ator) induced myopathy in adult male albino rats. Materials and Methods: Thirty six adult male albino rats were divided into four groups: group I (control group 12 rats). group }} (8 rats): receivedAtor (10mg/kg/day). group }}} (8 rats) and group IV (8 rats),both groups received Ator as in group }} concomitantly with CoQ10 (100mg/kg/day) orAlfacalcidol(0.5æg/kg/day) respectively. After each expermintal period 4 and 8 weeks, 6 rats from the control group and 4 rats from groups (II, III & IV) were sacrificed. Blood samples werecollected and analyzed for creatine phosphokinase (CPK). Sections from the middle part of biceps femoris muscle were processed for histological(H.&E.),immunohistochemical staining for Cytochrome C immunoreactivity, as well assemithin examination, morphometric and statistical studies. Results: After 4 weeks,H.&E. and semithinsections of group II revealed disorganized, fragmented and tapered muscle fibers. Many fibers in transverse section acquired an irregular outline rather than polygonal. Most of them were surrounded by a halo of clear external rims. Also, elevated CPK level and mean area % of Cytochrome C immunoreactivity, and decreased mean muscle fiber diameter were recorded. However, group III showed partial improvement of muscle fibers damage, decrease in CPK level andmean area % of Cytochrome C immunoreactivity and increase in muscle fiber diameter. While, group IV showed prominent improvement proved histologically and morphometrically versus group III. After 8 weeks, muscle damage involved wider areas in group II. group III revealed remarkable amelioration of muscle tissue damage when compared to group IV
