Abstract

Background: Multifocal development of transitional cell carcinoma (TCC) in the entire urinary tract is well known. TCC of the bladder sometimes involves the prostate. Prostatic TCC may involve urethral mucosa,duct,acini and stroma of the gland. Primary TCC of the prostatic urethral mucosa is rare,but development of TCC in the PU during follow-up of bladder cancer patients is more common The exact incidence of mucosal involvement of the PU in patients with superficial bladder TCC is not known well and the degree of involvement of the prostate has been shown to affect the outcome Aim of the work: In this study we evaluated the risk factors for mucosal prostatic urethral involvement in primary bladder mass. Patients and methodology: this study done on 100 patients with bladder mass ,all patients were free urethra from tumor ,biopsy was taken from prostatic urethra at 5,7 o,clock and send for histopathology result. Results: prostatic urethra was positive in 25 patients(25%). The most important factor that increase the incidence of protatic urethra involvement is PR,hydronephrosis on CT, gross appearance,PR under anesthesia,stage of tumor with P value(0.004, 0.005, 0.018, 0.025, 0.037) respectively. Conclusion: The current study shows that prostatic urothelial carcinoma is a frequent finding in bladder cancer (especially deep muscle invasive bladder cancer )that is not visible by cystoscopy and diagnosed by TUR biopsy from prostatic urethra . routine prostatic urethral sampling particularly in patients suspected to have invasive muscle layer tumor should be considered which may have an impact on further management of these patients(in patients planning cystectomy, prostatic urethra carcinoma involvement may change the type of diversions)
