Abstract

Uterine operations are common procedures in gynecology as myomectomy and in obstetrics as caesarean sections. The type of suture material has an impact on the inflammatory reaction and the integrity of the tissue after the healing process. So, aim of this work was to study the effect suturing material and size of the suture on uterine scar healing. Material and methods: 23 white New Zealand strain rabbits were divided randomly into 2 experimental groups; 10 rabbits each and a control group (3 rabbits). In rabbits of group I, an incision of 1cm length was done each uterine horn. The right horn incision was sutured using Vicryl of thickness size 1. The left horn incision was sutured using Monocryl of thickness size 1. Half of the animals were sacrificed after 1 week and the other half after 3 weeks. The same incision was done in rabbits of group II, but the right horn incision was sutured using Vicryl of thickness size 2/0 and the left horn incision was using Monocryl of thickness size 2/0. Half of the animals were sacrificed after 1 week and the other half after 3 weeks. Results: Residual myometrial thickness at the site of the healing incision was significantly higher after using Monocryl as compared to Vicryl. Inflammatory reaction was more marked after using Vicryl and more prominent after using thick sutures as inflammatory index was highest one week after using Vicryl 1 and least after using Monocryl 2/0. The amount of fibrosis and fibroblast growth factor (FGF) expression were significantly higher after using Vicryl as compared to Monocryl. Vascular endothelial growth factor (VEGF)expression was higher after using Monocryl as compared to Vicryl. Conclusion: Monocryl sutures gave better healing results than Vicryl sutures. Moreover, thinner sutures give better healing results than thick sutures in the form of less inflammatory reaction and less collagen deposition.
