Abstract

Deviated nasal septum is one of the most common nasal structural defects and a prevalent problem among the general population worldwide. Septoplasty is a commonly performed surgery for the correction of a symptomatic deviated nasal septum.Nasal packing is routinely done as a last step of this surgery with the purpose of preventing postoperative bleeding, formation of synechiae or septal hematoma, optimizing the position of the septal flaps, and closing the dead space between cartilage and mucoperichondrial flaps. Various recent studies advocate for transseptal suturing technique as an effective alternative as it can prevent postoperative hematoma and bleeding without the complications associated with nasal packing.Trans-septal sutures can also eliminate pain, anxiety, and discomfort related to nasal packing and increase the stability of the septum and minimize complications.This prospective comparative study was conducted on 60 patients above 18years underwent septal surgery, for the correction of symptomatic deviated nasal septum. Patients were randomly divided into 2 groups; each group consists of 30 patients. group A for trans-septal suture technique without nasal packing and group B for nasal packing with Merocel after septal correction surgery. Postoperative subjective and objective assessment of all patients was conducted in three follow up visits 48 hours, 2 weeks and 8 weeks postoperative. Comparison of the results was conducted between both groups.The results of the study demonstrated a significant difference between both groups. Patients of transseptal suturing group reported much less postoperative pain, headache, epiphora and sleep disturbance in comparison to nasal packing group. Transseptal suturing group also showed a significant decrease in postoperative nasal crustation compared to nasal packing group
