Abstract

PCT is considered as an early biomarker for diagnosis of bacterial infections in advanced hepatic diseases, and is considered as a good indicator when compared to c- reactive protein (CRP), tumor necrosis factor (TNF) alpha or proinflammatory interleukins 6 and 2. PCT is considered as a significant marker for sepsis because its level is usually high in septic conditions but remains low in nonspecific inflammatory diseases and viral infections, so serum PCT has a diagnostic role in SBP and bacterial infections in advanced hepatic disease. Aim: The aim of the present study is to assess the validity of serum and ascitic fluid procalcitonin in diagnosis and optimization of antibiotic therapy in spontaneous bacterial peritonitis in cirrhotic patients. Methods: This study is a prospective cross sectional one that involved 55 patients with liver cirrhosis and ascites. 25 of them suffered from SBP and the other 30 patients were free from bacterial infection and considered as a control group. The patients included in study were admitted to specialized medical hospital (SMH), Mansoura university from January 2020 till January 2021.Results: significant positive correlation between ascitic fluid WBCS count and ascitic fluid PCT (r=0.295, p=0.032) and ascitic fluid WBCS and serum PCT (r=0.325, p=0.019). serum PCT is statistically significant predictor of SBP among studied cases with every increase one unit increases risk of SBP by 1.01 (odds ratio= 1.01, 95% CI: 1.0 -1.01). The overall percent predicted is 87.3%.Summary/Conclusion: Our results noted that serum and ascitic fluid PCT are good markers in predicting SBP and differentiating SBP from non SBP patients, and there is a positive correlation between serum and ascitic fluid PCT and ascitic fluid WBCS.
