Abstract

In 1988,Claude Franceschi described Conservative Hemodynamic cure of Venous Insufficiency in an Ambulatory basis, known by the French acronym ``CHIVA’’. As one of treatment options of varicose veins .(5)
CHIVA was designed to allow treatment of varicose veins without sacrificing the superficial vein network, and consists of minimally invasive surgical procedures that are based on findings of careful haemodynamic analysis of the venous network of the lower limb using duplex imaging.
CHIVA is based on the fact that the physiological route of venous blood is an anterograde circuit, draining from the superficial venous system to the deep venous system, and finally, to the heart. Primary varicose veins are characterized by a retrograde circuit or a venous-venous shunt .
The aim of the CHIVA treatment is to eliminate the venous-venous shunts by disconnecting the escape points, preserving the saphenous vein for future use as a graft and normal venous drainage of the superficial tissues of the limb.
After the study of CHIVA method we found that it was superior to other general methods in treating lower limb varicose veins as it has better surgery effect , small incision ,quick recovery ,high safety factor and low recurrence rate.
Successful CHIVA requires significant trained and expert surgeon as well as a complete understanding of the underlying venous hemodynamic principles. Ay inadequate understanding of CHIVA may produce worse results than traditional surgeries. CHIVA also needs accurate duplex ultrasound mapping and physician
