Abstract

Summary
This thesis is a comparative study between diagnostic and therapeutic value of both ascending urethrogram and urethral ultrasound in anterior urethral stricture.
80 male patients with symptoms suggesting obstructive symptoms were included in this study.
All patients were examined clinically with verification of the past history of trauma, previous operations, and recurrent infection.
Preoperative evaluation for all the patients include complete urine analysis culture and sensitivity , kidney and liver function tests, complete blood picture, abdominal and pelvic ultrasound.
After definitive treatment either by visual urethrotomy or excision and end to end anastmosis; follow up done after 3 and 6 months
The age of patients was between 18 and 40, kidney function test was normal in 70% of cases.
The main cause of stricture urethra was inflammatory in 35% of cases. The difference in the length of stricture as measured by ascending urethrogram and urethral ultrasound varied from 3 to 8 mm more with urethral ultrasound.
The number of strictures in the same patients was between 2 and 4 in (60%) of cases.
Visual internal urethrotomy was done in 72 patients, excision and end to end anastmosis was done in 8 patients.
It was found also that if the length of the stricture < 18mm: the cure rate was (50%) in cases treated with VIU and recurrent stricture was found in (14.2 %) of cases treated with VIU.
But if the length of the strictured segment was > 18mm the cure rate was (25%) in those treated by VIU and recurrent stricture was found in (50 %) of cases treated by VIU.
Urethral ultrasound proves the presence of spongiofibrosis in all cases (100%) and in no cases with ascending urethrogram.
Urethral ultrasound proved the presence of stricture in (100%) while ascending urethrogram diagnosed stricture in (95%) of cases.
