Abstract

The present study was performed to evaluate both clinically and radiographically the effect of platelet rich fibrin (PRF) alone and combined autogenous bone graft on healing of jaw bone defects following the enucleation of maxillary cysts.
This study was conducted on twenty one patients who were referred to the Oral and Maxillofacial Surgery Department, Faculty of Dentistry, Alexandria University, and their age ranging from 20 to 40 years old. They were free from any relevant disease that would contraindicate surgical intervention or affect the healing of the surgical wound.
Under local or general anesthesia, a mucoperiosteal incision was performed to create a flap for sufficient visualization and management. After reflection of the flap, enucleation of the whole pathological tissue, complete curettage and irrigation with sterile normal saline were performed.
Apicectomy was performed for the teeth involved in the lesion. Thereafter,in patients of group A the defects were filled with PRF with ABG, while patients of group B the defects were filled with PRF alone.While patients of group C the defects were left without any graft material. Finally, repositioning of the flap and sutures were performed.
Post-operative antibiotics and analgesics were prescribed, proper oral hygiene was maintained by instructing the patients to use warm mouth wash and soft diet was recommended for one week postoperatively.
The follow-up phase was performed both clinically and radiographically as follows:
- Clinically on the third & seventh day and weekly for the first month post-operatively for the presence of pain, swelling, infection, or any other complication.
- CT scan was performed for all patients immediately post-operative and after six months to evaluate the surface area of the bone defects and the density of newly formed bone.
The obtained data were statistically analyzed, tabulated and represented by graphs.
6.1.1 Clinical results
All patients recovered well after surgery, they showed no complications throughout the evaluation period. None of the patients showed any signs of infection or inflammation throughout the study period.
6.1.2 Radiographic evaluation
The bone density was measured in Hounsfield Units by the aid of RadiAnt DICOM Viewer software. Radiographic evaluation revealed a significant decrease in the size of bone defects in both groups. Regarding bone density, it increased significantly at six months interval when compared to the baseline in both groups.
Comparing both groups, group A showed significantly higher bone density and more bone formation than group B and C, which suggests a superior effect of PRF when used in association with Autogenous Bone Graft when using PRF alone.
