Abstract

Patient safety is defined as freedom from accidental or preventable injuries produced by medical care. And world health organization (WHO) also provided the following definitions: “patient safety is a discipline in the healthcare sector that applies safety towards the goal of achieving a trustworthy system of health care delivery”. Patient safety is an attribute of health care system that minimize the incidence and impact of recovery from adverse events (World Health Organization (WHO). 2017).
Generally technical errors were the most frequent cause of preventable adverse events followed by failure to request or arrange investigation or procedure, failure to synthesize, decide or act on information and lack of care. Inexperience and communication errors were the two most frequent types of system failure, while technical errors and cognitive failure were the most common types of error. Lack of experience, inadequate supervision of junior staff and problems in communications were the most common types of system errors (Mueller et al, 2019).
The present study aimed at assessing challenges facing staff nurses to maintain patient safety.
The study was conducted in cardiovascular hospital affiliated to Ain Shams University hospitals using a descriptive correlational research design. The subjects of the present study included all available staff nurses working in cardiovascular hospital affiliated to Ain Shams University hospitals, they were 120 staff nurses. 10% of them (12) were included in the pilot study, and also included in the main study sample.
The tool used for collecting data of this study was a self-administered questionnaire sheet was developed by the researcher based on pertainant literatures (Hwang et al, 2012-Muntlin et al, 2010- Currie and Watterson, 2007) to assess challenges facing staff nurses to maintain patient safety. It was consisted of two parts, the first part “Demographic characteristics of the study sample” and the second part “Patient safety challenges questionnaire sheet” to assess patient safety challenges as perceived by staff nurses. It consisted of 63 items with responses on a 3-point Likert scale agree, uncertain, disagree.
The findings of the current study can be summarized as:
• (51.7%) of the studied staff nurses had ages (20 ≤ 30) year, (56.7%) of them had Technical Institute of Nursing. Also, (46.7%) of the studied staff nurses had years of experience (6 ≤ 10) years. Finally, (60%) of the studied staff nurses didn’t attend training courses related to patient safety. While, (40%) of them were attended training courses related to patient safety.
• (60%) of the studied staff nurses had high level of work environment related challenges. Also, (56.7%) of them had high level of task and technology related challenges. Moreover, (35.8%) of them had moderate level of teamwork related challenges.
• .)55%( of the studied staff nurses had high level of total challenges facing them to maintain patient safety. Also (30 %( of them had moderate level of total challenges. While, (15 %( of them had low level of total challenges.
Based on the findings of the current study, it was concluded that more than half of the studied staff nurses had high level of total challenges facing them to maintain patient safety and there was highly statistically positive correlation between patient related challenges and teamwork related challenges, task and technology related challenges and institutional context related challenges.
It was recommended to conduct training programs for staff nurses to improve knowledge and practices related to patient safety, conduct educational programs for patients about their safety, rights and responsibilities, developing handover system for staff nurses and training them for applying it, providing training program for nurses about analyzing and documentation of risks, considering the patient safety concept as part of the nurses orientation training program and making sure about availability of preventive equipment and supplies for nurses to minimize the risk occurrence.
