Abstract

Patients undergoing cardiothoracic surgery report slightly higher incidence of oxygenation defects and pain intensity than other surgery types. Aim of the study: is to evaluate the effect of immediate chest physiotherapy and pain management on oxygenation level and pulmonary function among early extubated patient post heart surgery. Design: Quasi experimental. Setting: cardiothoracic ICU and cardiothoracic department at cardiothoracic hospital that belongs to Minia University Hospitals, Egypt. Three tools: Socio demographic and medical assessment tool, oxygenation level assessment tool and Numeric Pain Scale. Results: mean age of study and control groups were (38.4±5.4) & (37.2±4.8) year respectively. (40%) from the study group suffered myocardial infarction and where did coronary angioplasty, (43.3%) of the control suffered coronary artery stenosis (33.3%) among them did coronary angioplasty. Realted to pain scale there was significant difference between study and control groups at p value <0.05 post surgery. Also regarding to oxygenation level results were presented by significant difference between both groups about breathing sound and ABGs at 1st, 3rd and 7th day after extubated, presented by p value <0.05. Conclusion: This study concluded that chest physiotherapy and pain management have a positive effect on patient’s oxygenation level and pulmonary function which was presented in improving patients breathing sound, pulmonary function items and arterial blood gases post heart surgery. Also there was statistically significant relation between chest pain intensity and pulmonary function of study group post-operative as TLC and FRC at (P= < 0.05). Recommendations: All cardiac surgery patients should receive an explanation and training about chest physiotherapy and pain management strategies before their operations to improve their health outcomes.
