Abstract

Hypertensive disorders are the most common medical complication in pregnancy, affecting approximately 5–10 % of all maternities. Despite advances in obstetric medicine, it remains the second highest cause of maternal mortality worldwide (Say, Chou, Gemmill, Tunçalp et al., 2014), as well as a major cause of morbidity for the mother and the baby (Lo, Mission and Caughey, 2013).The present study was carried out to assess the effect of clinical practice guidelines on the performance of nurses caring for women with eclampsia and pre eclampsia. It was conducted at menoufia University Hospital and Shebin El-Kom Teaching Hospital. Research hyposis was formulated and quasi experimental design was used in carrying out the current study.The researcher used 160 nurses as a sample size. The tool used in the data collection process was developed by the researcher then revised by qualified experts. It was a structured interviewing questionnaire which included soci - demographic data, a questionnaire of three point likert scale was used to evaluate nurses Knowledge about pre-eclampsia and eclampsia, a questionnaire for evaluating nurses’ knowledge about the clinical practice guidelines of preeclampsia and eclampsia and Observational checklist for evaluating nurses’ application of clinical practice guidelines.Human rights and ethical considerations were observed and considered in conducting the study. An oral consent to participate in the current study was taken after the purpose of the study was clearly explained to each subjects. Confidentiality of obtained personal data as well as respect of participants’ privacy was totally insured.The study had certain limitations, related to the subjects who included in the study. They were nurses who work in obstetrics and gynecological departments and operation room.The main findings of the current study were as follows: As regards to socio-demographic characteristics of the studied nurses, there were about 62.5% were married. Nearly 43.8% of the studied nurses have bachelor in nursing and the same were diploma. About 55% work in operating room of obstetrics and gynecology. Whereas the majority of them hadn’t attended training courses about care preeclampsia and eclampsia. And about 66.3% of the studied nurses dealt with cases of preeclampsia and eclampsia. Concerning the nurse’s level of knowledge about preeclampsia, there was a highly statistical significant difference between the nurses’ pre and post intervention regarding their knowledge about preeclampsia. The current study shows that the nurse’s level of knowledge about eclampsia. was a highly statistical significant difference between the nurses’ pre and post intervention regarding their knowledge about eclampsia. The study findings revealed that the total knowledge mean score of the studied nurses about preeclampsia and eclampsia in the pre, post intervention was highly statistical significant difference between the nurses’ pre and post intervention regarding their total knowledge about preeclampsia and eclampsia. As regards to diagnosing preeclampsia and eclampsia , there was a highly statistical significant difference between the pre, post intervention regarding nurses’ knowledge and performance about clinical practice guidelines of diagnosing preeclampsia andeclampsia. Also there is no statistical differences between post intervention and after 3 months intervention. There was a highly statistical significant difference between the pre, post intervention regarding nurses’ knowledge and performance about clinical practice guidelines of care of women with preeclampsia. Also there is no statistical differences between posttest and after 3 months intervention. The study showed that there was highly statistical significant difference between the pre, post intervention regarding their performance about clinical practice guidelines of care of women with HELLP syndrome as a complication of preeclampsia and eclampsia. Also there is a statistical difference between post and after 3 months intervention regarding nurses’ knowledge and performance about clinical practice guidelines of care of women with HELLP syndrome as a complication of preeclampsia and eclampsia. The study finding revealed that there was highly statistical significant difference between the pre, post and after 3 months intervention regarding nurses’ knowledge and performance about clinical practice guidelines of care of women with eclampsia. Also there is no statistical differences between post test and after 3 months intervention. Concerning clinical practice guidelines of Care of women after delivery, there was a highly statistical significant difference between the pre, post and after 3 months intervention regarding nurses’ knowledge and performance about clinical practice guidelines of care of women with preeclampsia and eclampsia after delivery. Also there is no statistical differences between post test and after 3 months intervention. The current study revealed that there was a highly statistical significant difference between the pre, post and after 3 months intervention regarding nurses’ knowledge and performance about clinical practice guidelines of care of women with preeclampsia and eclampsia after discharge. Also there is no statistical differences between post test and after 3 months intervention. Concerning the level of total clinical practice guidelines about preeclampsia and eclampsia, there was a highly statistical significant difference between the pre, post and after 3 months intervention regarding nurses’ knowledge and level of total performance about clinical practice guidelines of care of women with preeclampsia and eclampsia. As regards to the Mean scores of total clinical practice about preeclampsia and eclampsia, there was a highly statistical significant difference between the pre, post and after 3 months intervention regarding nurses’ knowledge and total performance about clinical practice guidelines of care of women with preeclampsia and eclampsia. The study finding revealed that there was statistical significant difference regarding total knowledge score in relation to nurses’ level of education in pretest. As nurses who have Bachelor of nursing have increased mean than those who have diploma or technical institute. Also there is no statistical differences in the post test regarding total knowledge score in relation to their level of education. Also there is a highly statistical difference in pretest regarding total practice guidelines in relation to their level of education. As nurses who have Bachelor of nursing have increased mean than those who have diploma or technical institute. There was statistical differences in the post test regarding total practice guidelines in relation to their level of education. There was no statistical differences in the follow up test regarding total practice guidelines in relation to their level of education. The current study showed that there was no statistical significant difference between the pre, post (total knowledge) and post and after 3 months intervention regarding nurses’ total practice guidelines of care of women with preeclampsia and eclampsia in relation to their working department. But there is high statistical significance in the pre test regarding total practice guidelines in relation to their working department. There was a highly statistical significant difference between the pre test of total knowledge score and pretest of total practice guidelines in relation to their years of experience. Also there is no statistical differences in the post test of total knowledge score and post test and follow up of total practice score in relation to their years of experience. The study findings revealed positive Correlation between total knowledge and total practice guidelines score. This mean that highly statistical significance between total knowledge score and total practice guidelines score.Therefore, it can be concluded that program about clinical practice guidelines of preeclampsia and eclampsia was effective in improving nurses’ knowledge and practice regarding care of preeclampsia and eclampsia.
