Abstract

Dry eye is a common disorder, with an estimated 25% of patients in general ophthalmology clinics reporting dry eye symptoms. Dry eye is diagnosed with validated patient symptom questionnaires and a wide array of clinical assessments of the tears and ocular surface. Currently, no cure for dry eye is known. Common treatments are targeted to management of symptoms. The mainstay of conventional therapy consists of application of artificial tears that increase moisture on the ocular surface and provide additional lubrication. Autologous serum eye drops have been found in clinical trials to be beneficial in patients who have persistent symptoms and signs. This aim of this study was to show the role of autologus serum eye drops in treatment of dry eye The study included 100eyes with severe dry eye from 50 patients recruited from Mansoura ophthalmic center. The cases were randomizely divided into two groups; the first group received autologous serum eye drops for treating dry eye while the other groups received artificial tears.All the included cases were subjected to full history taking (including ophthalmic history) and full phthalmological examination. Before treatment evaluation of lacrimal tear and ocular surface was done using Tear film break up time and Schirmer test. Tear meniscus height was performed using Swept-source anterior segment optical coherence tomography instrument (Triton, Topcon). The patients within the two groups ;( group A consist of 50 eyes that received autologous serum eye drops combined with artificial tears for treating sever dry eye and group B consists of the contralateral 50 eyes that received only artificial tears substitutes for treating sever dry eye) were followed up on the first month and at first 2 months after treatment with artificial tears alone or artificial tears with autologous serum.The results of this study showed that:1There was a statistically significant increase in the TMH after treatment in the two study groups as compared to the pretreatment value with no statistically significant difference between the two groups.2.There was a statistically significant increase in the value of Schirmer test after treatment in the two study groups as compared to the pretreatment value. Also the group received autologous serum drops showed significant improvement as compared with the other group.3.There was a statistically significant increase in the value of TBUT after treatment in the two study groups as compared to the pretreatment value. Also the group received autologous serum drops showed significant improvement as compared with the other group.4.There was no statistically significant change in the rose Bengal stain in the two study groups as compared to the pretreatment value and there was no statistically significant difference between the two groups before and after treatment. Based on the results of our study, it could be included that:•Dry eye is a common disease affecting the quality of life in the affected cases especially in the old age. •Different treatment modalities are available, but it mainly concerned with controlling the symptoms of the disease and not treating the cause. •The use of autologous serum eye drops was associated with significant improvement of the symptoms and different parameters of dry eye disease.1.Good ocular care especially in old age for avoiding the development of dry eye mptoms.2.Comparing the effects of autologous serum eye drops with other available treatment regimen to get more accurat sults.3.Further studies should be performed including larger number of patients from more than one center.4.Further studies should be conducted to obtain other treatment modalities for treatment of dry eye.
