Abstract

Ototoxicity is the functional impairment of the inner ear due to cellular degeneration caused by therapeutic agents leading to loss of hearing and/or vestibular function. The most common ototoxic drugs are aminoglycoside antibiotics e.g. gentamicin, platinumbased cytostatic drugs, non-steroidal anti-inflammatory drugs& loop diuretics. Gentamicin is one of the aminoglycoside antibiotics widely used to treat gram -ve bacterial infections and resistant TB. Its use has been relatively decreased due to appearance of other broad-spectrum antibiotics and its dangerous side effects (nephrotoxicity and irreversible ototoxicity).Nevertheless, appearance of multi-drug-resistant organisms in multiple types of infections e.g. urinary tract infections and tuberculosis, has kept aminoglycoside antibiotics in consistently significant indications. Gentamicin ototoxicity is usually bilateral, symmetrical and its severity depends on dose and duration. Hearing impairment firstly affects the higher pitch sounds and after long duration it affects lower pitch sounds. It affects mainly the organ of corti and the hair cells especially at the basal part of the cochlea.
