Abstract Splenomegaly is common clinical finding In hematological diseases especially in inherited hemoglobin disorders as thalassemia. Laparoscopic splenectomy (LS) is the gold standard management of most splenic pathologies in which medical therapy fails to control the symptoms or complications. However ,splenomegaly could be a challenging task for a laparoscopic removal. In this study, we explored the feasibility of LS in patients with splenomegaly using a laparoscopic approach in patients with thalassemia . This is a prospective case series of 30 patients (7–24 years old) who were operated from November 2017 till august 2019 with splenomegaly because of thalassemia. All of the patients were operated under general anesthesia after preoperative investigations , preparations and appropriated criteria . By the use of the harmonic scalpel and the ligasure it is become more feasible and increase efficacy of laparoscopic splenectomy as a line of treatment in patients with splenic disorders . In spite of relative more operative time of laparoscopic splenectomy, yet the main advantages of laparoscopic splenectomy is less intra operative bleeding, less hospitalization and hence early return to work and employment as compared to open splenectomy. LS in patients with thalassemia is feasible and safe in experienced hands. |
