Abstract

In an attempt to assess the ability of ultrasound to accurately predict the
mode of delivery This prospective cohort study was conducted at the
department of Obstetrics & Gynecology at Om El Masryeen general
Hospital, Giza, Egypt., from November 2018 to March 2019. Eighty
primgravida women were recruited, with gestational age of 37 to 40 weeks,
singleton fetus, cephalic with intact membranes. All women were in active
phase of first stage of labor.
We excluded women with Multiple pregnancy, Rupture of membranes,
Induced labor, Fetal presentation other than cephalic and Obstetric
indication for CS e.g. CPD, fetal distress...etc. After taking history and
physical examination patients were submitted for transperineal Ultrasound
imaging to measure the AOP and HPD, the mode of delivery was followed
to test the ability of the measured parameters to predict the mode of delivery.
Among the recruited cases; 58 cases delivered vaginally (VD) and 22
cases delivered by caesarean section (CS). We found that for a cut off < 4.5
cm head–perineum distance correctly identified cases who successfully
delivered vaginally with a sensitivity of about 88% and a specificity of 91%.
And at cut off > 104 º, angle of progression correctly identified cases who
successfully delivered vaginally with a sensitivity of about 90% and a
specificity of 86%.
Trans-labial ultrasound can accurately predict the mode of delivery
providing a more scientific basis for assessing labor.
