Abstract
This study was carried out at Menoufiya University Hospitals
and Ain shams University Hospitals The study was performed to
determine whether performing TVS before hysteroscopy will reduce
the use of diagnostic hysteroscopies for evaluation of uterine cavity
pathologies. TVS and hysteroscopy was done for 60 women with
primary and secondary infertility to diagnose uterine cavity
abnormalities.
The evaluation was scheduled during the follicular phase
(postmenstrual) days 5 to 10. from this study, the normal endometrial
findings by hysteroscopy was lower than TVS (% 55 versus 75%)
while the total pathological findings by hysteroscopy was higher than
TVS (45% vesus 25%).
Concerning this study, TVS diagnose 8 cases with polyps
missed 3 cases, 5 cases of fibroids missed 2 cases, 2 cases of IUA,
However, diagnostic hysteroscopy did not miss any of these cases.
The diagnostic performance of TVS for endometrial polyps
were 73.3%, 89.8%, 70.3%, 84.2% respectively for TVS as regarding
the sensitivity, specificity, positive predictive value (PPV) and
negative predictive value (NPV) . For submucus fibroids, the
diagnostic performance of TVS were (81.4%, 94.3%, 80%, 90%
.However, the diagnostic performance of TVS for intrauterine
adhesions was (50%, 100%, 100%, 96.5%).
Hysteroscopy was the gold standard diagnostic tool in
diagnosis, with 100% sensitivity and 100% specificity for detection of
all uterine pathologies. No complication had occurred to any case,
patient is then informed if they need any other intervention before IVF
or not.
