Abstract

Childhood-onset Systemic Lupus Erythematosus is a lifelong autoimmune disease that may be difficult to diagnose due to its multisystem involvement, and heterogeneity of clinical manifestations. It follows aggressive disease course than adult-onset SLE, with greater disease activity at presentation and consequently leads to greater morbidity and mortality. Moreover children with SLE have challenging time of life when physical appearance is important, self-esteem and identity are developed, also responsibility of decision making for their disease are frequent. Recognition of these specific issues is key to optimal disease management of children with SLE. To care effectively for a lupus child, the nurse and other health professional team need an up-to-date knowledge and practice related to disease. The result from this study will better determine the long-term outcome of children and may form the basis of nursing management and treatment approach for children with SLE. The current study aimed to assess the perception and practice of nurses related to health problems accompanying of children with Systemic Lupus Erythematosus. A descriptive research design was used in the study .The study was conducted at inpatient nephrology unit of pediatric department at Tanta and El-Mansora University main Hospital. A convenient sampling consisted of eighty nurses were selected from the setting mentioned before. Tools of data collection: Two tools were used in the study. Tool I: Structured interview schedule and sociodemographic characteristics: It was developed by the researcher after reviewing of literature to obtain the following information Part (1): Sociodemographic characteristics of the studied Nurses related to age, educational level, residence, marital status. Part (2): Demographic characteristics of the children related to age, sex, birth order. Part (3): Medical history of children such as: Auto immune diseases, bleeding and Genetic disorder. Part (4): Nurses’ perception about children with Systemic Lupus Erythematosus disease regarding the following items : Definition, causes, pathophysiology, clinical manifestation, complications, management, prognosis and nursing management of children. Part (5): Common health problems accompanying child with Systemic Lupus Erythematosus such as: Fever, malar Rash, oral ulcers, anemia, weight loss, photo sensitivity, activity intolerance, painful joint and anxiety. Tool II: Nurses Practice observational checklist related to health problems of child with Systemic Lupus Erythematosus which include the following : - Physical assessment to monitor growth and development. - Skin care of malar rash. - Mouth care of oral ulcers. - Infection control for prevention of secondary infection related to corticosteroids drugs. - Laboratory investigation for early detection of another system disorder. - Monitoring vital signs to maintain homodynamic stability. - Fluids intake and output monitoring for early detection and treatment of renal disorder. - Administration of blood transfusion if needed as in anemia and bleeding. - Administration of medication to reduce joint pain as ordered as in arthritis. - Emotional support to reduce anxiety and depression. - Follow up schedule.
