Abstract

This study was made on 100 non obstrstructive azoospermic / cryptozoospermic patients to show the effect of Aromatase in hibitors on TESE results on non obstructive a zoospermic patients .
These patients were selected according to clinical examination , semen analysis and hormonal profile suggestive of NoA .
These patients were classified into 2 groups :-
group ( A ) : 50 non obstructive a zoospermic patients were subjected to the following :
1- Full informed cansent .
2- Full history taking .
3- Full general and andrological examination at the same time of presentation .
4- Full hormonal profile .
5- Repeated semen analysis .
6- Micro TESE .
group ( B ) : The other 50 non obstructive a zoospermic patients performed the same investigations like group ( A ) but in addition they have taken treatment in the form of Aromatase inhibitors ( letrozole ) ( 2.5 mg daily for 3 months ) before TESE trial .
The results showed the following in group ( A ) : 50 non obstructive a zoospermic patients . All of them have done TESE . TESE SRR was 50 % .
group ( B ) : The other 50 patients who were divided into 31 non obstructive azoospermic patients and 19 severe oligo / cryptozoospermic patients . These patients were given Letrozole ( 2.5 mg daily for 3 months ) after which 12 azoospermic patients showed sperm in the ejaculate and 19 remained azoospermic . Thus thesevere oligo / cryptozoospermic became 31 patients after treatment . However at day of Intra cytoplasmic sperm injection . 12 patients from the severe oligo / cryptozoospermic recesitated TESE trial to retrieve sufficient sperm for ICSI . So 31 cases have done TESE . SRR was ( 35.5 ) ( 11 / 31 ) and 19 cases remained severe oligo / cryptozoospermia not recesitating TESE trial . 6 of them were non obstructive azoospermic and became severe oligo / cryptozoospermic patients .
Regarding TESE SRR between 2 groups theres was non significant statistical difference ( P value = 0.0.252 ) .
Conclusion:
Treatment with Aromatase inhibitors ( AI ) may save non obstructive azoospermic patients from TESE procedure in up to ( 19 % ) as 6 / 31 not recesitating TESE trial . However it doesn’t improve sperm Recovery from TESE . A larger prospective Randomised study is Recommended to confirm this interesting finding.
