أولاً- مشكلة الدراسة:
يعتبر المجال النفسي مجالًا حيويًا وهامًا من مجالات الخدمة الاجتماعية والذى يمارس فى المؤسسات الطبية النفسية، لمساعدة المرضى للتغلب على الصعوبات والأعراض النفسية التى تعوق أدائهم لأدوارهم الاجتماعية من خلال التعاون مع فريق العمل لتقديم خدمات متكاملة للمرضى.
و يمكن صياغة مشكلة الدراسة في العنوان التالي ” فاعلية فريق العمل فى تقديم رعاية متكاملة للمرضى النفسيين من منظور الممارسة العامة فى الخدمة الاجتماعية ”.
ثانياً- أهمية الدراسة:
تحددت أهمية الدراسة في النقاط التالية:
⦁ التطور الظاهر فى مجال الطب النفسى، والنظرة الشمولية للمرضى النفسيين والتى تؤكد على أن المرض النفسى لا يرتبط بالأبعاد الذاتية للمريض، ولكن أيضا للظروف البيئية التى يعيش فيها، وشكل التفاعل بين الإنسان والبيئة مما دعا إلى الحاجة لفريق عمل يهتم بكافة الأبعاد المرتبطة بالمرضى النفسيين.
⦁ ان مجال الطب النفسى أصبح لا يركز فقط على تقديم العلاج النفسى فقط، ولكنه يتضمن تقديم خدمات متكاملة للمرضى تتضمن البعد النفسى والاجتماعى والروحانى، حيث أن الخدمات المتكاملة تسهم فى فعالية العلاج المقدم للمرضى.
⦁ أصبح العصر الحديث وما يضمه من تقدم فى العلوم الإنسانية والاجتماعية يميل إلى التخصصية فى التعامل مع الاضطرابات والمشكلات والضغوط مما يؤكد على أهمية التعامل بفريق العمل فى تقديم خدمات متكاملة للمرضى.
⦁ يعتبر مجال الطب النفسى من المجالات الثانوية لممارسة مهنة الخدمة الاجتماعية، وهذا المجال يحظى باهتمام المهنة منذ نشأتها الأولى والتى كانت تهتم بالاضطرابات النفسية والسلوكية، وبالتالى فان المهنة يتوافر لديها العديد من النماذج والتى يمكن أن تُستخدم مع المرضى فى هذا المجال، لمساعدتهم على التوافق النفسى والاجتماعى.
ثالثاً- أهداف الدراسة:
تحددت أهداف الدراسة في النقاط التالية:
أولاً: تحديد مستوى كفاءة الأداء المهنى لفريق العمل فى تقديم رعاية متكاملة للمرضى النفسيين. ويتم تحديد ذلك من خلال المؤشرات الآتية:-
⦁ تحديد مستوى وضوح الأهداف لدى فريق العمل.
⦁ تحديد مستوى الالتزام بالقواعد والمعايير الأخلاقية فى فريق العمل مع المرضى النفسيين.
⦁ تحديد مستوي أداء أدوار كل تخصص داخل فريق العمل مع المرضى النفسيين.
⦁ تحديد نوعية الخدمات التى يقدمها فريق العمل للمرضى النفسيين.
⦁ تحديد مدي توافر الموارد الفنية والمادية لفريق العمل.
ثانياً: تحديد مستوى فعالية فريق العمل فى تقديم رعاية متكاملة للمرضى النفسيين.
ويتم تحديد ذلك من خلال المؤشرات الآتية:-
⦁ تحديد مستوي الخدمات التي تقدمها المستشفى للمرضى النفسيين.
⦁ تحديد مستوى الاستمرارية فى تقديم الخدمات للمرضى النفسيين.
⦁ تحديد مستوى تأثير الخدمات على المرضى النفسيين.
⦁ تحديد مدى مراعاة الاعتبارات الإنسانية فى الخدمات المقدمة للمرضى النفسيين.
ثالثاً: التوصل لتصور المقترح لزيادة فعالية خدمات الرعاية المتكاملة للمرضى النفسيين فى إطار فريق العمل.
رابعاً- تساؤلات الدراسة:
تحددت تساؤلات الدراسة في الآتي:
التساؤل الأول: ما مستوى كفاءة الأداء المهنى لفريق العمل فى تقديم رعاية متكاملة للمرضى النفسيين؟ ويشتمل هذا التساؤل على مجموعة تساؤلات فرعية وهى كالآتى:-
⦁ ما مستوى وضوح الأهداف لدى فريق العمل؟
⦁ ما مستوى الالتزام بالقواعد والمعايير الأخلاقية فى فريق العمل مع المرضى النفسيين؟
⦁ ما مستوي أداء أدوار كل تخصص داخل فريق العمل مع المرضى النفسيين؟
⦁ ما نوعية الخدمات التى يقدمها فريق العمل للمرضى النفسيين ؟
⦁ ما مدي توافر الموارد الفنية والمادية لفريق العمل ؟
التساؤل الثانى: ما مستوى فعالية فريق العمل فى تقديم رعاية متكاملة للمرضى النفسيين؟ ويشتمل هذا التساؤل على مجموعة تساؤلات فرعية وهى كالآتى:-
⦁ ما مستوي الخدمات التي يقدمها فريق العمل للمرضى النفسيين ؟
⦁ ما مستوى الاستمرارية فى تقديم الخدمات للمرضى النفسيين؟
⦁ ما مستوى تأثير الخدمات على المرضى النفسيين ؟
⦁ ما مدى مراعاة الاعتبارات الإنسانية فى تقديم الخدمات المقدمة للمرضى النفسيين؟
خامساً- الإجراءات المنهجية للدراسة:
أولاً- نوع الدراسة: تنتمي هذه الدراسة إلى نمط الدراسات الوصفية حيث تهتم هذه الدراسة بوصف وتحليل الخدمات التى يقدمها فريق العمل للمرضى النفسيين، ومدى إستمرارية تقديم تلك الخدمات وتأثيرها والأعتبارات الإنسانية فى تقديمها للمرضى النفسيين، وتحديد أهداف فريق العمل والقواعد التى يعمل فى ضوئها وأدوار كل عضوء داخل فريق العمل ونوعيه الخدمات التى يقدمها الفريق للمرضى ومن ثم التوصل إلى تصور مقترح من منظور الممارسة العامة في الخدمة الاجتماعية لزيادة فعالية فريق العمل فى تقديم الرعاية المتكاملة للمرضى النفسيين.
ثانياً- المنهج المستخدم: استعان الباحث في دراسته بمنهج المسح الاجتماعي والذي يعد أحد المناهج الرئيسة التي تستخدم في البحوث الوصفية.
⦁ المسح الاجتماعي الشامل للأخصائيين الاجتماعيين والنفسيين، والصيادلة.
⦁ المسح الاجتماعي بالعينة للأطباء النفسيين والتمريض.
ثالثاً- أدوات الدراسة:
⦁ أدوات جمع البيانات:
اعتمد الباحث في هذه الدراسة على الأدوات الآتية:
⦁ مقياس لفريق العمل حول كفاءة الأداء المهني لفريق العمل مع المرضي النفسيين.
⦁ مقياس لفعالية فريق العمل فى تقديم خدمات الرعاية المتكاملة للمرضي النفسيين.
رابعاً- مجالات الدراسة:
⦁ المجال المكاني: وتحدد المجال المكاني لهذه الدراسة مستشفى الأمراض النفسية والعصبية وجراحة المخ والأعصاب الجامعى بأسيوط .
⦁ المجال البشرى: وتم اختيار المجال البشرى لهذه الدراسة وفقا للآتي:
⦁ المرضى النفسيين تم أخذ عينة من المرضى النفسيين المترددين على العيادات الخارجية وعددهم80 مفردة.
⦁ فريق العمل بالمستشفى متمثل فى الطبيب النفسى والأخصائى الأجتماعى والنفسى والصيادلة والتمريض وعددهم 23 مفردة.
⦁ المجال الزمني: وهى فترة جمع البيانات من الميدان وهى الفترة من 20/2/2018 حتى 26/4/2018م.
خامساً- نتائج الدراسة:
توصلت نتائج الدراسة إلى الاتى:
⦁ أثبتت نتائج الدراسة أن أهم أهداف فريق العمل مع المرضى النفسيين أنه يسعى لعلاج المرضى بإقناعهم بمدى أهمية العلاج، وكذلك يتعاون جميع أعضاء الفريق فى تنفيذ الخطة العلاجية التى تم وضعها، حيث يقوموا بتقديم خدمات الأرشاد النفسى للمرضى وتخفيف الأعراض التى يعانون منها.
⦁ أثبتت نتائج الدراسة أن أهم القواعد والمعايير الأخلاقية التى يتبعها فريق العمل مع المرضى النفسيين هى إحترام كل عضو لتخصص باقى أعضاء الفريق، إدراك كل عضو من أعضاء الفريق لمسؤلياتة تجاه المرضى النفسيين، ووجود هدف واضح مشترك لدى فريق العمل، والتكامل بين أدوار أعضاء فريق العمل لصالح المريض.
⦁ أثبتت نتائج الدراسة وجود مجموعة من الأدوار التي يمارسها الطبيب النفسى ضمن فريق العمل وتمثلت هذه الأدوار في الآتي تشخيص نوعية المرض النفسى ، تحديد نوعية العلاج الذى يتناسب مع طبيعة كل حالة ، تقديم العلاج للمريض النفسى فى الوقت المناسب ، أما الأدوار التي يمارسها الأخصائى النفسى تمثلت في الآتي إجراء الاختبارات النفسية للمرضى، مساعدة المريض النفسى على مواجهة مشاكله النفسية، وتقديم المشورة النفسية للمرضي، المشاركة فى التأهيل النفسى للمريض، أما الأدوار التي يمارسها الأخصائى الاجتماعى تمثلت فى الآتي إعادة تأهيل المرضي للعودة للحياة الأجتماعية، إمداد أعضاء الفريق بمعلومات عن الظروف الاجتماعية للمرضي، ويحدد خطة الأنشطة الاجتماعية التى يتم تنفيذها للمرضى.
⦁ أثبتت نتائج الدراسة وجود مجموعة من الخدمات التي يقدمها فريق العمل، وكان من أهم الخدمات النفسية تقدم الجلسات الكهربية للمرضى النفسيين، تتوافر الأدوية النفسية التى يحتاج لها المرضى، وتقديم الدعم النفسي للمرضي، وأهم الخدمات الأجتماعية وجود أماكن لممارسة الأنشطة الاجتماعية، التواصل مع أسرة المرضى النفسيين، وكان من أهم الخدمات الصحية وجود طبيب مقيم داخل المستشفى، ةوتوافر التخصصات المختلفة من الأطباء بالمستشفى، وتقديم الإسعافات الأولية للمرضى عند الإصابة، ومن أهم الخدمات الترفيهية شغل وقت المريض داخل المستشفى بشكل يفيد العلاج، توفير الإمكانيات اللازمة لممارسة المرضي للأنشطة الرياضية.
⦁ توصلت نتائج الدراسة إلى أن أهم الموارد الفنية والمادية لفريق العمل فهم كل تخصص لدور التخصص الآخر، تواجد أماكن مناسبة للقيام بالعمل الجماعى، وعقد دورات لتنمية مهارات فريق العمل.
⦁ أثبتت نتائج الدراسة وجود مجموعة من الخدمات التى يقدمها فريق العمل للمرضى النفسيين، ومن أهم الخدمات التى يقدمها الطبيب النفسي، يقوم بالفحص الطبى للمريض فور وصوله المستشفى، يقدم العلاج النفسي لكل حالة ، وتشخيص نوعية المرض لكل حالة، أما أهم الخدمات التى يقدمها الأخصائى النفسي، يقلل من إنفعالات المريض النفسية، يساعد المريض على مواجهة مشاكله النفسية، وتقديم المشورة النفسية لكل مريض، أما الخدمات التى يقدمها الأخصائى الأجتماعى، يساعد الأسرة فى فهم طبيعة المرض النفسي، يساعد تلمريض على مواجهة مشاكله الأجتماعية، ويقوم بإعادة تأهيل المرضى.
⦁ توصلت نتائج الدراسة إلى أن الاستمرارية فى تقديم الخدمات للمرضى النفسيين تمثلت فى الآتي تنظيف العنابر بإستمرار، صرف العلاج بإنتظام للمرضى، والحصول على الخدمات فى الوقت المحدد.
⦁ أثبتت نتائج الدراسة أن تأثير خدمات فريق العمل جاء كالآتي تحسن حالة المرضى عند دخولهم المستشفى، أصبح المرضى قادرون على الأعتماد على أنفسهم، تغير احساس الخوف من الاخرين، و أصبحت قادراً علي التعامل مع الآخرين.
⦁ توصلت الدراسة إلى أن أهم الاعتبارات الإنسانية فى تقديم الخدمات للمرضى النفسيين كالآتي، تحدث الطبيب مع المرضى بلغة تناسب ثقافتهم، إستماع الطبيب للمريض عند حديثهُ معه، تعامل التمريض مع المرضى بعطف وإحترام، وحفظ المعلومات الخاصة بالمرضى.
⦁ توصلت الدراسة إلى تصور مقترح من منظور الممارسة العامة للخدمة الاجتماعية لزيادة فعالية فريق العمل في تقديم خدمات الرعاية المتكاملة للمرضى النفسيين .
Summary of the Study
I- The problem of the study
Psychological field is a vital and important field of social Work, which is practiced in Psychiatry institutions, to help patients overcome the difficulties and psychological symptoms that impede their performance of their social roles through cooperation with the team to provide integrated services for patients.The problem of the study can be framed in the title
“The Effectiveness of The team work in provided integral care For
Psychiatric Patients from the perspective of generalist social work practice ”.
II- Importance of the study
The importance of the study was identified in the following points:
1- The apparent development in the field of psychiatry, and the comprehensive view of Psychiatric patients, which emphasizes that the Psychological illness is not related to the dimensions of the subjective patient, but also to the environmental conditions in which he lives, and the form of interaction between humans and the environment, which called for the need for a team concerned with all dimensions associated with Psychiatric patients.
2- The field of psychiatry is not only focused on the provision of psychotherapy, but it includes the provision of comprehensive services to patients including the psychological, social and spiritual dimension, as integrated services contribute to the effectiveness of treatment provided to patients.
3- The modern era and its progress in the human and social sciences has become specialized in dealing with disorders, problems and pressures, which emphasizes the importance of dealing with the team in providing integrated services for patients.
4- The field of psychiatry is considered one of the secondary fields for practicing the profession of social Work. This field has been of interest to the profession since its inception, which was concerned with Psychological and behavioral disorders. Therefore, the profession has many models that can be used with patients in this field to help them. Psychological and social compatibility.
III- The study Objectives
The objectives of the study were identified in the following points:
First: Determine the level of efficiency of the professional performance of the team in providing integrated care for psychiatric patients.
This is determined by the following indicators:
⦁ Determine the level of clarity of objectives in the team.
⦁ Determine the level of adherence to ethical rules and standards in the team work with psychiatric patients.
⦁ Determine the Type of the roles of each specialization within the working Team with psychiatric patients.
⦁ Determine the quality of services provided by the team work to psychiatric patients.
⦁ Identify the technical and material resources of the team.
Second: Determine the level of effectiveness of the team in providing integrated care for psychiatric patients.
This is determined by the following indicators:
⦁ Determine the quality of the services provided by the team work to psychiatric patients.
⦁ Determine the level of continuity in the provision of services for psychiatric patients.
⦁ Determine the level of impact of services on psychiatric patients.
⦁ Determination of humanitarian considerations in services provided to psychiatric patients.
Third: Reach a suggested perception to increase the effectiveness Integrated care services for psychiatric patients within the framework of the Team work.

The Questions of the study were identified as the following:
The First question: What is the level of efficiency of the professional performance of the team in providing integrated care for psychiatric patients? This question includes a set of sub-questions as follows:

V- Methodological procedures of the study
The type of study:
The current study belongs to the pattern of descriptive studies by which we can determine the properties of a particular phenomenon or a particular situation, This study deals with the description and analysis of the services provided by the team work to psychiatric patients, and the continuity of providing these services and their impact and the humanitarian considerations in presenting them to the psychiatric patients. Team members and the type of services provided by the team to the patients, and then to Reach a suggested perception to increase the effectiveness Integrated care services for psychiatric patients within the framework of the Team work.
The Used methodology:
The researcher in this study used social survey approach, which is one of the main approaches used in descriptive research.
⦁ Comprehensive social survey of social workers, psychologists and pharmacists.
⦁ Sample social survey for psychiatrists and nursing.

The researcher in this study depended on the following tools:
⦁ A measure of the team work on the efficiency of the professional performance of the team with psychologists.
⦁ A measure of the effectiveness of the team in providing integrated psychiatric care services.
Fields of the study:
⦁ Spatial field:
The spatial domain of this study determined in the hospital of psychiatric and neurological diseases and neurosurgery in Assiut.
The human field:
The human field of this study was selected according to the following:
⦁ First: Psychiatric patients A sample of ( 80 ) outpatient psychopaths was taken.
⦁ Second: • The team working in the hospital is represented by the psychiatrist, social worker, psychologist, pharmacists and nursing, the number of (23) individuals.
⦁ Time domain field: is the period of data collection from the field which was the period from 20/2/2018 to 26/4/2018.
VI- Results of the study
The results of the study represented in the following:
1- The results of the study proved that the most important goals of the team with Psychiatric patients is that it seeks to treat the patients by convincing them of the importance of the treatment, and also cooperate with all the members of the team in the implementation of the treatment plan that has been developed, where they provide psychological counseling services for patients and alleviate their symptoms.
2- The results of the study proved that the most important rules and ethical standards of the team working with Psychiatric patients is the respect of each member to specialize the rest of the team members, the recognition of each member of the team for responsibility for Psychiatric patients, and the existence of a clear goal shared by the team and the integration of the roles of team members Work for the patient.
3- The results of the study proved the existence of a set of roles practiced by the psychiatrist within the work team. These roles were as follows: Diagnosis of the quality of the Psychological illness, determining the type of treatment that is appropriate to the nature of each case, providing the treatment to the Psychiatric patient in a timely manner. The roles practiced by the psychologist The following are the psychological tests for the patients, helping the Psychiatric patient to face his psychological problems, providing psychological counseling to the patients, participating in the psychological rehabilitation of the patient. The roles practiced by the social worker were as follows: rehabilitation of the patients to return to social life , Provide team members with information on the social conditions of patients, and identify the plan of social activities implemented for patients.
4- The results of the study proved that there is Many services provided by the team work. The most important psychological services are the psychological sessions for the Psychological ill, the psychological drugs needed by the patients, psychological support for the patients, the most important social services, the places to practice social activities, Psychiatric patients. One of the most important health services was the presence of a resident doctor in the hospital. Different specialties are available from doctors in the hospital. First aid is provided to patients at the time of injury. Intentions necessary for the exercise of satisfactory sports activities.
5 - The results of the study proved that the most important technical and material resources of the team work to understand each specialty of the role of the other specialization, the presence of suitable places to do the collective work, and to hold courses to develop the skills of the team work.
6- The results of the study proved the existence of a range of services provided by the mental health team. The most important services provided by the psychiatrist are the medical examination of the patient upon his arrival to the hospital. He provides psychological treatment for each case and diagnoses the quality of the disease for each case. The most important services provided by the specialist Psychological, reduces the patient’s emotional emotions, helps the patient to face his psychological problems, and provide psychological counseling to each patient. The services provided by the social worker, helps the family to understand the nature of the psychological illness, helps the patient to face his social problems and rehabilitates the patients.
7 - The results of the study proved that the continuity in the provision of services for psychiatric patients was the following cleaning of the barns constantly, the treatment of patients regularly, and access services on time.
8- The results of the study proved that the impact of the services of the team work is as the improvement of the condition of patients when they enter the hospital, the patients are able to rely on themselves, change the sense of fear of others, and become able to deal with others.
9- The study reached to the most important humanitarian considerations in the provision of services for psychiatric patients as follows, the doctor spoke with patients in language appropriate to their culture, listening to the patient when talking to him, treating patients with patient sympathy and respect, and the preservation of patient information.
10- The study proposed a suggested perception from the perspective of the generalist social work practice to to increase the effectiveness Integrated care services for psychiatric patients.
