Abstract This study was conducted on 40 patients presented with malignant endobronchial neoplasms, with a mean age of (51.23±10.39) years and the percentage of male to female patients where (77.5 %) and (22.5%) respectively. All patients had symptoms related to central airway obstruction. The main symptoms were dyspnea, cough, and hemoptysis. All patients were subjected to bronchoscopic depulking where 20 patients were treated with EC and another 20 patients were treated with CRYO, in order to evaluate the role of both interventional modalities in management of central airway obstruction. Comorbidity was present in 26 out of 40 studied patients (65%) where11 (55%) out of 26 patients were included in CRYO group and 15 (75%) patients were included in EC group. The type of histopathological diagnosis among total population in this study where non small cell carcinoma was present in 21 out of 40 studied cases while small-cell lung cancer was present in 11 patients of both groups. The results revealed that both therapeutic interventional modalities were successful, not only in reconstruction of the patency of the airways in patients with endobronchial tumors with evident reinflation of the collapsed part of the lung radiologically, but also, in controlling of symptoms (dyspnea, cough and hemoptysis) related to the obstruction with concomitant objective improvement in ABGs and pulmonary function tests with a positive impact on quality of life of such patients.
