Abstract

Metal phosphide poisoning has become an emergent health problem in many countries including Egypt. With the advance of times, these are nowadays routinely used for modern cultivation method, readily available as over the counter preparations in village shops and act as a common agent for suicidal purpose after trivial family problems. The high rate of metal phosphide poisoning can be attributed to the uncontrolled trade of this material, its inappropriate conservation, high efficacy and low cost, widespread use by the public and consequent availability. Appropriate use of these compounds, proper labeling and usage instructions, information about their harmful effects and restriction of their uncontrolled sales by legal regulations can reduce phosphides poisoning incidence. Social, domestic and economic problems are increasing; ultimately creating psychological disturbances. Such stress may force a person to commit suicide by using metal phosphides due to its low price, high toxicity and easy availability. After ingestion or inhalation of metal phosphide phosphine gas is formed. Zinc phosphide requires acidic conditions for appreciable hydrolysis and subsequent formation of phosphine, whereas aluminum phosphide and magnesium phosphide will hydrolyze to form phosphine in neutral pH. Phosphine gas evolved causes denaturing of various enzymes involved in cellular respiration and metabolism, blocks the enzyme cytochrome C oxidase as a result of which mitochondrial oxidative phosphorylation is inhibited, Phosphine is responsible for the denaturation of oxyhemoglobin molecule.
