Abstract

Background: The main issue in the practice of regional anesthesia is the ability to deliver the right dose of local anesthetic to a target nerve without incurring risk of nerve damage or its related structures.
Aim of the Work: The aim of this study is to compare the efficacy, number of trials, complications and volume of local anesthetics when peripheral nerve or plexus blocks are performed under guidance of nerve stimulator alone (NS), ultrasound alone (USG) or NS and US together (Dual guidance).
Patients and Methods: This prospective randomized controlled trial (RCT) was done at Ain Shams university hospitals, included 90 patients from both sexes aged 18 years and older and scheduled for upper limb surgeries under interscalene and axillary brachial plexus blocks. These patients were randomly and manually divided into 2 equal sized groups (each consists of 45 patients), each group is divided into 3 subgroups (each consists of 15 patients).
Conclusion and Recommendations: The use of dual guidance in brachial plexus block is implemented to improve and support the quality of the block as it provides a mean for proper visualization and neuro stimulation at the same time aiming to improve block success.
