Abstract Introduction: Echocardiography of the right ventricle and assessment of its both systolic and diastolic function is an important issue which is related significantly with coronary artery disease and its grade of severity and also which coronary artery involved so the study of the correlation between both is considered as important subject especially to know the echocardiographic parameters which may predict later on the severity of coronary artery disease. Objective: The purpose of the present study was to determine which parameters of right ventricular function assessed by tissue Doppler imaging in echocardiography will correlate more with the severity of coronary artery disease on coronary angiography which assessed by gensini and syntax II scores. Material and Methods: 60 patients were enrolled in this prospective cross sectional observational study, (mean age was 52.25 ± 7.82 years, 42 males), all patients underwent coronary angiography and in Mansoura specialized medical hospital over a period of 1 year from October 2015 to October 2016, All patients were assessed by thorough history taking, clinical examination,12 lead surface ECG, Echocardiography, coronary angiography and angiographic assessment of coronary artery disease. Results: There was statistically significant decrease in tissue Doppler measures of (S) wave velocity in patients with severe coronary artery disease (P=> 0.001). Also the color-coded tissue Doppler reveals significant decrease in S` wave velocities at the level of the basal and mid segments of the RV free wall (P=0.013, and P=0.026 respectively). There was statistically significant decrease in tissue Doppler measures of (S) wave velocities in patients with severe coronary artery disease (P=> 0.001). Our study revealed statistically significant increase in RMPI by using both DTI and color coded tissue Doppler imaging in patients with high angiographic Gensini score (P= 0.032 and P=> 0.001 respectively). Also, there is significant increase in value of RMPI by DTI and color coded tissue Doppler imaging with increasing syntax score (P=0.036 and P=0.004 respectively). Our study revealed statistically significant decrease in S`/RMPI ratio measured by PWD and also by Color coded DTI in severe coronary artery disease via Gensini score (P= 0.032 and P=0.012 respectively).Also it was decreased in severe coronary artery disease assessed by syntax score (P=> 0.001 and P=0.004). In our study, there is significant strong negative correlation between the number of vessel affected and PWD S` and S`/RMPI. Summary and Conclusion: The RVMPI , Doppler tissue velocities PW-DTI and S`/RMPI ratio by both CC DTI and by PWD showed significant changes in their values in patients with severe coronary artery disease and patients presenting with AMI especially that involving the right ventricle. |
