Abstract

ARF and RHD still constitute a major health problem in developing countries in spite of the fact that the disease prevalence has markedly regressed in western and industrialized countries. Consequently and with only a few notable exceptions, ARF and RHD are now largely diseases of developing countries and of underprivileged and indigenous populations of developed countries. In these areas, pattern of valvular lesions can still be detected. Mitral valve lesion is the most frequent heart valve disease followed by aortic valve lesion that is usually accompanied by mitral valve lesion. Tricuspid valve is less commonly affected with no pulmonary valve involvement.
Aim of the work:
The aim of the present work was to evaluate the pattern of valvular lesions in children diagnosed to have rheumatic heart disease. The study covered patients having follow-up at the pediatric cardiology clinic in Alexandria University Children’s Hospital (AUCH).
Subject and methods:
In our study, we reviewed the files of 109 patients previously diagnosed with RHD who attend our cardiology outpatient clinic in AUCH and we followed them up for the past 2 years to detect the pattern of valvular lesions. Fifty three of our patients are males (48.6%) and 56 are females (51.4%). Clinical examination was performed during each follow-up visit and different diagnostic tools were used to regularly assess their condition e.g. lab tests, CXR, ECG and echocardiography.
Results:
Of the 109 children included in our study, 107 patients (98.2%) 0f the patients had mitral valve lesion being the most frequent heart valve disease, all of them presented by mitral regurgitation. Only 1 patient had mild MS and another patient had moderate MS at the end of the study. Aortic valve involvement occurred in 51 patients (46.8%) 0f the patients, multiple left sided lesion occurred in 49 patients (44.9%) 0f the patients and Pure aortic regurgitation occurred in 2 patients (1.8%) of the patients. Tricuspid regurgitation was present in 16 patients (14.7 %) 0f the patients with no pulmonary valve involvement.
