Abstract

Contradictory results were reported in literature when the density of Tregulatory cells(T regs)in the skin of mycosis fungoides (a type of cutaneous T cell lymphoma) was compared with some pathologically mimicking inflammatory dermatoses.The aim of this study is to assess the density of FoxP3+ Treg cells in lesional skin of MF and compare the density Tregs to its pathological mimickers (psoriasiform, spongiotic and lichenoid dermatitis), in addition, to compare the density of Tregs to the clinical and pathological presentation of MF.This is a comparative cross-sectional study on lesional skin biopsies from 36 cases of mycosis fungoides versus 36 cases of histopathological mimickers including 10 cases of psoriasis, 10 cases of spongiotic dermatitis and 16 cases of lichenoiddermatoses (10 with PLC and 6 with DLE). We compared the densities of Foxp3+ CD25+ Tregs in relation to CD4+ cells in the epidermis and dermis between these groups using Foxp3, CD25 and CD4 monoclonal antibodies.MF group showed a lower statistical significant difference regarding epidermal and dermal FoxP3/CD4+ and CD25/CD4+ ratios than histologic mimickers group. Plaque MF had higher densities of regulatory T cells than patch stages. MF lesions showed a significantly lower density of CD4+CD25+Foxp3+ Tregs compared to its histological mimickers which might be attributed to their limited proliferation at the expense of malignant T lymphocytes. The increased density in plaque more than patch stage support the theory of increased Tregs recruitment to the tumor microenvironment
