Abstract

Summary and Conclusion
C
omparison of Von Willebrand factor antigen (vWF-Ag) in the serum in the 2 groups by ELISA method and found significant rise with the advance of the CLD.
The study was done on 40 subjects from Ain Shams University hospital were enrolled into the study in the period from July 2013 to February 2014, the subjects were classified into two groups:
Group I: 20 patients diagnosed with liver cirrhosis of any grade presented with hematemesis and undergoing band ligation of OV.
Group II: 20 normal control during routine clinical pre-employment assessment.
There was a highly negative correlation between serum Von Willebr and factor antigen (vWF-Ag) and albumin, while it was highly positive with S.bilirubin, Child, MELD and INR. No correlation with Hb, Plt, PV diameter, size of the spleen, AST and ALT.
We found high statistical significance Regarding comparison between vWF level and grade III and IV OV. Using ROC curve , we found that diagnostic performance of vWF level for discriminating high risk OV ( grade III and IV) was >367% in cirrhotic bleeder patients in order to predict those patients for performing upper GIT endoscopy to reduce risk of rebleeding and to avoid unnecessary endoscopy .
Regarding comparison between levels of vWF and Child Pugh score There is highly significant correlation with the severity of the chronic liver disease as P < 0.001.
So, the conclusion is that that the mean results of vWF before band ligation was 420.5%, after 1 week was 401.7% and after 6 weeks was 401.2%. So there was a high statistical significance between vWF level before and after 1 week from band ligation (as P < 0.05) and there was a high statistical significance between vWF level before and after 6 weeks from band ligation (as P < 0.05). But there is no statistical significance between vWF level after 1 week and after 6 weeks from band ligation (as p > 0.05)
