Abstract

The present study was carried out in EL-MINIA UNIVERSITY HOSPITAL- DEP. OF OBST&GYN ., during the period from the first of April 2002 to the end of December 2002 aiming to study maternal and neonatal mortality and morbidity and to focus on the contributing factors of maternal and neonatal mortality and morbidity.
The present study included 2737 cases admitted in the hospital and also included 2799 neonate delivered during the study period .In the present study PG were 1013 and MG were 1724 case .
Spontaneous vaginal delivery was the commonest method of delivery where it constitute 1797 cases from total deliveries 2737.Caesarean section was done in 477 cases from total 2737cases ( 17.5%) during this period of time. Instrumental deliveries by Ventouse were (22 case) and forceps were (1 7 case) . This indicate that instrumental deliveries decreased and the new trend tend to decrease using this instruments (safe vaginal delivery or caesarean section as rule) .
Anaemia was the commonest maternal morbidity observed during the study period (22 case ), followed by ruptured uterus 12 case, puerperal sepsis 10 cases occurred after discharge the patient from the hospital , chest infection 3 cases occurred after discharge from the hospital .Infected episiotomy 5 cases , other morbidities as heart failure , perineal tears , fistula, DVT , were 2 case only for each morbidity .
In the present study the MMR was (292 /100,000live births) .Eight cases of maternal mortality occurred during the study period and the commonest cause of maternal death was atonic postpartum haemorrhage 2 cases and ruptured uterus 2 cases . Cardiac diseases , eclampsia, sepsis and DIC each reported one case for each of them
The cardiac case died interapartum, four cases died during first day after delivery ,two cases died during first week after delivery and one case only died during end of the first month after deliver
In the present study (147) cases of neonatal morbidity was recorded (52/ 1000 live births ) during the study period, the commonest causes of neonatal morbidity were congenital anomalies 47 cases , chronic lung diseases was the second neonatal morbidity in the present study( 33) cases from 2799 neonates. Other morbidities was noticed as Erb , s , liver diseases , sepsis neonatorum, umbilical sepsis and torticollis.
In the present study 146 case of neonatal mortality occurred including 37 case of congenital anomalies (52/100,000 live births) , the corrected neonatal mortality rate after excluding cases of congenital anomalies was (109 case of neonatal death ) { corrected neonatal mortality was 40 /1000 live births} .The commonest causes of neonatal death were prematurity 59 case (40.5%) from total 2799 neonates during this period and the second causes of neonatal death were the congenital.
