Abstract

This study was conducted to evaluate the effect of constructing overdentures using the neutral zone technique on the success of immediate loaded unsplinted mandibular implants, using a flapless approach. Also to determine the difference in implants positioning following neutral zone technique verses traditional technique.
Twelve completely edentulous patients were selected for this study with age ranging from 48 to 65 years (mean 56 years)
Patients participated in this study, were free from any systemic or intra-oral diseases that may compromise implant success.
Twenty fourimplants were placed in the anterior mandibular region. Six patienthave their overdenture according to traditional manner (Group I), and the other six have their overdenture according to the neutral zone technique (Group II).
Each patient received two screw shaped, one-piece, similarly designed mini-implants with the same length (13mm) and diameter (3.8mm) in the interforaminal region of the mandible using a flapless surgical technique.
Clinical evaluation was performed at 1, 3, 6, and 9 months after implants placement and denture insertion in terms of plaque index (PI), gingival index (GI), probing depth (PD) and mobility.
Radiographic evaluation was performed immediately, 1, 3, 6 and 9 months after implants placement including assessment of marginal peri-implant bone level change and peri-implant bone density.
All parameters were measured by the same investigator.
Clinical results showed a significant decrease in PI, GI scores, increase in PD, and absence of mobility through interval periods and insignificant between the two groups
