Abstract

Aim of the work
To compare between 6 of the most commonly used methods of management of finger tip injuries which are:
1- Conservative treatment by open dressings.
2- Split-thickness skin graft.
3- Full-thickness skin graft.
4- V-Y advancement flap.
5- Cross finger flap.
6- Thenar flap.
Conclusion
On conclusion, we found that the age of the patients has an important role in the frequency of injury as most of finger tip injuries affected middle aged individuals. Also the occupation has a direct relation to the frequency of injyry, as most patients were butchers and factory workers.
Open dressing provides excellent healing, color match, finger tip contour and a very good 2 PD but it needs a relatively longer time than other methods to achieve its end result, but in general most patients were satisfied.
Grafts (STSG and FTSG) have unsatisfactory color match, contour (especially the STSG) and 2 PD with a high rate of post operative pain tenderness. Also grafts can be infected more easily than flaps. However it is rapid and easy method of treatment.
V-Y advancement flaps provide a rapid one-stage procedure with good contour, color match and 2 PD. But needs meticulous flap dissection to avoid flap loss that is not uncommon.
Cross-finger and thenar. Flaps are very reliable methods that can provide a sufficient amount of tissue to cover a big defect with accepted contour, color match, 2 PD and post operative pain and tenderness. They have a very minimal rate of flap loss. But they are a two-stage operation, which found objection from some patients. In old age patients it may cause joint stiffness.
